Shelter Yetu is seeking to fill the position of Case Manager as detailed below;
Job title: Case Manager (Social Work)
Purpose: To ensure efficient and effective day to day running of social activities in the project and the welfare of all beneficiaries (direct and indirect) are well observed. This should be done in line with Shelter policies on beneficiary protection and care within the delegated authority of a results based management framework.
Reports to: Head of Social Work
Job Goal:
Assist in rescue, rehabilitation, reintegration and follow up of street-connected children, with a focus on case management.
Conducting initial assessments of the child’s situation to determine needs and goals
Conducting family empowerment, counseling and safe reunification and follow ups until the client reaches the age of 18
Recording and reporting child progress and updating children files
Addressing each case as a unit and setting tailored measurable goals
Conduct home/school visits for reintegrated boys and update the administrators and write reports on the same
With the guidance and approval of the Head of Social Work, rescue and rehabilitate boys in the streets as described by the Shelter Yetu’s Rescue and Rehabilitation Manual
Create and maintain relationships with stakeholders in a professional manner
Advocate for family based care in the local community, alongside the administrator and directors
Work with local authorities and extended family members of beneficiaries to promote family rescued boys to prepare for reintegration
Conduct counseling sessions to children, parents and extended family members to enhance rehabilitation and reintegration
Recommend temporary foster care and advise foster or adoptive parents
Preferred Requirements and experience:
Diploma in Social work or related field.
Computer literacy: working knowledge of MS office required
Good knowledge of children rights and abuse prevention
Strong interpersonal and communication skills
